If you are a group/organisation looking to make a booking, please email the following information to email@example.com.
- Name of Group/Organisation
- Requested Booking Date
- Requested Booking Times
- Requested Activity(ies)
- Estimated # of Guests
- Range of Disabilities within the Guests Attending
Once received, we will confirm availability and if available, email you a Confirmation Booking Invoice (the $ amount on this invoice will based on your estimated # of guests) You should then complete the following forms:
- Health & Consent Form (1 per person). The link to this form should be sent to each guest/caregiver/parent to complete. If this option is not possible, a group leader may complete this form instead of the guest/caregiver by completing the following form: Group Consent & Assessment Form (1 form per 10 people)
- Waiver (1 form per person). This form must be completed by a guest to caregiver/parent.
Once your forms have been received, we will review the information and update your Confirmation Booking Invoice with any updated charges (if relevant). Payment may be made online by credit card or by cheque. Cheques are payable to Rocky Mountain Adaptive and may be mailed to 2-201 Carey, Canmore, AB, T1W 2R7.
